For your convenience, I have available a Trauma Symptom Survey which will allow you to assess yourself for the possible symptoms of trauma.   

There are many ways trauma can show up in a person’s thoughts, feelings, and behaviors. Click the link below to open a survey that covers a variety of the common symptoms people have if they have been traumatized. The total score of the survey is not important. It’s more about discovering in what ways your trauma manifests itself in your life. The survey uses a 1-5 scale. One of the ways people can use this survey is to fill it out when you start therapy. Six months later, fill it out a second time to see where things have changed, and what might need more attention.
